The exact application form will vary between deaneries and specialties but in reality many are similar in the style of questions asked. There are often two main sections. Firstly, there are standardised questions asking for generic information. These are used simply for administrative purposes. Any information regarding ethnicity, disability and sexual orientation may be asked for on a specifi c part of the form but this will not be seen by the shortlisting committee and indeed there is often a box marked ‘not disclosed’ if you do not wish to answer such information.
