Case 47 A new arrival with cough and a fever

History A 40-year-old man arrives in the United Kingdom from Bangladesh to visit relatives. He has been complaining of feeling generally unwell with a cough and fever for around 3 weeks, which is worsening. He attends the accident and emergency department complaining of a productive cough and feels increasingly systemically unwell with fevers and sweating. He has has been well previously with no smoking history or history of exposure to dust.
