ABSTRACT

History A 74-year-old man with a 40-year history of smoking attends his general practitioner (GP). He has been coughing up blood-stained sputum over the past 2 months and has also noted some weight loss. Over the past 48 hours he has noticed increasing shortness of breath and some discomfort in the left side of his chest.
