Introduction of the Primary Trocar A significant percentage of the major complications with laparoscopic surgery are associated with the insertion of the primary trocar through which the laparoscope will be introduced. More than half of these bowel lesions are not recognized during the procedure, when they are gastrointestinal or urinary, and diagnosed postoperatively when the patient returns with an acute abdomen as a result of peritonitis.
