Significant risk factors for vascular disease, includ-◾ ing obesity, smoking, male, middle age, hypertension, hyperlipidemia, probable obstructive sleep apnea, and a family history of vascular disease Sudden onset of global aphasia, dense weakness ◾ of the right lower face, arm and leg, hyperreflexia, Babinski sign, and right-sided sensory loss Conjugate eye deviation to the left side ◾ Lack of visual attention to the right visual field and ◾ loss of sensation on the right side of the body

As the clinical history suggests, Etienne’s predisposing features and clinical deficits suggest a disorder of the cerebral circulation . Subsequent discussion with respect to ischemic cerebrovascular disease must be based on an understanding of the mechanisms of cell death due to ischemia and a detailed knowledge of the blood supply to the brain . The effects of disturbances of that blood supply on different areas of the brain and their clinical effects will be examined .
