There are human error problems that go far beyond the simple errors attributed to one or two managers (
managerial errors
). Pervasive human error problems may afflict one company (
organizational error
) and result in loss of market share, financial viability, customer satisfaction, and adaptability to competitive forces. The human errors may spread over a group of companies or a trade group and can be considered
institutional errors
.
