ABSTRACT

Human error is an undesirable aspect of everyday life. It is often excused on the basis that mistakes are just part of the everyday learning process. Simply put, people make mistakes, to err is human, and people are not perfect, except possibly you. In general, it is to be expected. Human error may become manifest in the form of human behavior or conduct that can be categorized as undesirable, unacceptable, careless, inattentive, forgetful, reckless, harmful, a miscommunication, human performance that is extreme in variability or beyond the limits of that expected, or an inappropriate form of risk-taking behavior. An error may be harmless, it may be detectable and correctable, it may have only nuisance value, or it may serve to predict future problems. It is certainly important if human error is a cause of 50 to 90% of all accidents, as is often concluded.
