ABSTRACT

A great deal of effort may be expended on human error detection, identification, delineation, and refinement, but no real correction of the error problem can be attained without some specific countermeasure to control, contain, reduce, or eliminate the risks. Throughout this book there are examples of human error problems and the various remedies that were or should have been utilized to minimize the risks. Those examples show that there are many countermeasures that could be selected and tailored to meet various specific needs in given sets of circumstances. This chapter will provide additional general principles and a virtual checklist of specific countermeasures.
