ABSTRACT

Transvaginal endoscopic exploration is intended to make exploration of the female internal organs as minimally invasive as possible without compromising the diagnostic accuracy of the investigation. It allows direct and easy access and visualization of the tubo-ovarian structures without supplementary manipulation. With the endoscope in the same axis as the tubo-ovarian structures, the total ovarian surface and the fossa ovarica can easily be accessed. In the absence of any manipulation, transvaginal laparoscopy allows inspection of different structures in their normal position and normal relation to each other. This allows direct observation of normal physiologic events at ovulation and ovum pickup. (Figure 24.1). Compared with standard laparoscopy, following this procedure most patients had minimal pain (mainly in the lower abdomen).
