ABSTRACT
The subject of medical error has seldom been discussed, either privately or publicly, until quite recently (1). Physicians and nurses have been reluctant to discuss their mistakes, feelings exacerbated by the adversarial climate produced by the threat of malpractice litigation. The public also preferred to believe that errors inmedical practice were rare, and seldom questioned their care.
