ABSTRACT

Many omissions of operations occur because the operator is simply not present or unable to react. A list of typical reasons for this is the following:

• Operator absent from the workplace • Operator hindered or obstructed • Incapacitation • Comfort call • Distraction • Attention failure • Action prevented or hindered • Overload • Priority error • Focus error

One cause of operational input being overlooked is that of the operator simply being absent. Case History 1.2 in Chapter 1 is of this type. There are others.
