ABOUT THE ROOT CAUSE ANALYSIS EXAMPLES This chapter illustrates use errors resulting from user interface shortcomings and shows design modifications that might help in preventing such errors. Our illustrative format is different from the more “cut and dried” one used to report root cause analysis in a usability test report or adverse event report (see Chapter 11). Here, we have taken a more visual approach that cites potential solutions to user interface design flaws, with the goal of making the book more helpful to readers who would like to read about solutions in addition to problems.
