A polymorphous vascular pattern and a structureless red or pink color do not represent specific features, they are highly indicative of a malignant tumor. Malignant vascular, adnexal and fibrous tissue tumors represent uncommon entities, several are associated with a worse prognosis, compared to the common nonmelanoma skin cancers. A malignant counterpart has been described for several benign adnexal tumors. Sebaceous tumors (ST) are adnexal tumors with sebaceous differentiation mainly found in the context of Muir-Torre syndrome. STs comprise sebaceous adenoma, sebaceoma and sebaceous carcinoma, and may be difficult to differentiate clinically and histopathologically. First, tumors with a central crater, dermoscopically characterized by elongated radial telangiectasias surrounding an opaque structureless ovoid white-yellow center, which are at times covered by blood crusts. Sebaceous carcinoma has been reported to show polymorphous vessels, a yellow background, and ulceration on dermoscopy. Trichilemmal carcinoma is a rare tumor that arises on sun-exposed areas, most commonly the face and ears.
