ABSTRACT
In the wake of the landmark 1999 Institute of Medicine report To Err Is Human: Building a Safer Health System, international attention has been focused on the safety of our patients. Since then, health care practitioners, administrators, patients, industry, and academia have put time, energy, intellect, and dollars into reducing the number of patients who die or are injured due to health care errors.
