C – Right Kocher A Kocher incision is made 3 cm below and parallel to the subcostal margin from the midline to the border of the rectus abdominus muscle. A rightsided Kocher incision is appropriate for an open cholecystectomy. Le-sided incisions are used for splenectomy. e incision cannot be extended medially, and if it is extended too far laterally, many intercostal nerves can be damaged. A rooop incision (or double Kocher incision) is made up of both a le and a right Kocher incision connected at the middle. It provides good access to the liver and spleen. It is also indicated for use in bilateral adrenalectomy, radical pancreatic and gastric surgery.
