ABSTRACT

The initial assessment will be by a named midwife to whom the woman has been introduced. Where indicated (see Chapter 1), the admitting midwife should refer to the doctor on duty, without delay. The antenatal clinic records and records of any admissions in the pregnancy should be reviewed and any special instructions noted. A plan of care should be devised with the woman, building on wishes and plans agreed during antenatal care. This plan should be flexible, and subject to review during labour. Document the reason for admission, the findings on assessment and the plan of care.
