ABSTRACT
It has become popular among those interested in safety and quality to take a “ systems view” of health care processes, but the views of the systems that follow are wishful and simplistic, based on rationalized, engineered systems that are quite dif ferent from the actual world of health care work “in the wild” (Hutchins, 1996).
