ABSTRACT
Although there have been many developments in aviation, such as improvements to technology, regulations, policies and training, aviation continues to operate with inherent risks and hazards, including the risk of human error. This is often due to:
1. Accepted risks and hazards. a. Cost. It would be too costly to eliminate them,
accounting for the initial mitigation and maintenance of that mitigation.
