It is critical that the QA program cover both the clinical as well as the physical aspects of treatment planning and delivery, as errors can occur from a myriad of sources in the clinical environment. Of possible errors, the most catastrophic are the errors made by a medical physicist (Figure 27.1). A quality audit addresses potential catastrophic errors and provides a comprehensive analysis with constructive criticisms to aid the supervising medical physicist in improving upon the overall QA program (British Institute of Radiology 1983, 1996; Hendee and Orton 2008).
