ABSTRACT

Any discussion of prostate cancer needs to begin with the acknowledgement that this is perhaps the area of greatest controversy in urology. These controversies stem from the fact that the incidence of prostate cancer is high in older men. However, some would argue that these men die ‘with not of’ prostate cancer. There is therefore concern amongst some that early diagnosis and radical treatment of prostate cancer has the potential for marked over-treatment, without significant benefit in survival or quality of life. The opposing argument would maintain that early diagnosis and radical treatment improves survival. It is not the scope of this chapter to deal with this controversy, but it is impossible to deal with CaP without acknowledging it.
