ABSTRACT
A confidential reporting system for major transfusion events-serious hazards of transfusion (SHOT)—was launched in 1996. Although blood transfusion is generally extremely safe it does have several potentially fatal hazards. All staff handling blood should be aware of the importance of correct identity of sample patient and blood bag at all stages.
