ABSTRACT

Learning from an accident is often presented as a major goal for the subsequent accident investigation process. The investigation report is an important outcome of an investigation process. The text of the report is a possible vehicle for transferring experiences gained from the investigators to a broader public. Seldom or never the prerequisites for learning is further analyzed in the reports. There seems to be an underlying assumption that revealing and describing the causal chain behind the incident inevitably leads to learning. The specification of subjects for learning may also be missing, or restricted to defining organizations that are expected to follow up on certain recommendations. We will claim that there are several challenges still not fully analyzed related to the use of texts from investigation reports for learning purposes.
