ABSTRACT

The latter are related to procedural or recovery actions, which contain cognitive tasks (detection, diagnosis), followed by execution tasks (recollection, action).

When an initiator event occurs, the operator performance consists of different phases (Rasmussen, 1986). Human errors in each phase are divided between Errors of Omission (EOO) and Errors of Commission (EOC). Omission errors are related to any human action that should have been performed, but which were not actually carried out. Commission errors are related to human actions that should not be performed, but which in fact are performed (Alvarenga et al. 2011).
