ABSTRACT
To some extent progress in engineering and technology is built upon lessons learned from failures. Failure provides a very strong inducement to re-examine the theory, the assumptions, the design, construction and operational history. If the results of the failure analysis are made public – and, regrettably, often they are not – other engineers can learn from these failures and reduce the risk of repeating the mistake.
