ABSTRACT

STEEEP represents health care value of the highest order. The logic of continuous improvement of processes is self-evident; however, the pace at which health care improvements are adopted is slow to the point of embarrassment. Why did it take two decades to achieve the routine delivery of aspirin to patients arriving at the hospital with chest pain?1 And, over a decade to “hardwire” hand washing between patient visits?2 The answers are rooted in organizational culture and openness to the adoption of innovations.
