ABSTRACT
This is population management for a specific segment of the population that has been defined by their disease or condition rather than their payment category or demographic. It assumes that everyone and anyone that walks through a hospital portal or any kind of entry point, for that matter, will be managed by a team that has the broad charge of simply doing a great job for the patients in their particular discipline. As a corollary, the hospital and the providers who share the responsibility also share the income and the upsides and downsides related to the patient population being managed.
