ABSTRACT
Histograms are one of my favorite graphical depictions of reality or test of reality. Here is what I mean. The Centers for Medicare & Medicaid Services (CMS) publishes data on every quarter that CMS receives from acute care healthcare organizations in an effort to help consumers choose based on quality. These measures are important being evidence based in their correlation with patient outcomes, if not the outcomes themselves, such as measures of mortality. High reliability is dependent on reliable measurement systems that are capable of accurately and precisely measuring performance over time.
