ABSTRACT

Mr. B is not an unusual patient in our health care system. Physicians might label him unmotivated or noncompliant, and while several of the specialists he saw in the hospital probably encouraged him to become engaged in his own health, he doesn’t know where to begin. Mr. B’s multiple complex conditions are causing him to miss work, leading his employer to note that he is becoming less productive. Managing his own health is overwhelming, leaving him confused and somewhat depressed. To most of his family and coworkers, he is not himself. To objective professional observers, he appears

very off-track with limited likelihood of experiencing positive health outcomes in his near future.
