ABSTRACT
Integrating the continuum of healthcare providers both vertically across different levels of care and horizontally within a level has been an ongoing trend during the last two decades. The degrees of success in these ventures, however, have varied greatly. Some emerging models are incorporating more than traditional health organizations and are embracing new methods of governance, structure, and financing to reinvent the integrated holistic healthcare system of the future.
