Once a physician graduates from medical school no one evaluates his performance unless it is overtly poor. No one determines the standard for good health care or requires that everyone in the provider sector measure up to standard. A good example of outcome measurement is in the Jacksonville, Fla. Emergency Medical Care System. As Americans we have been concerned with good health and long life. The long-term care of our citizens has not been satisfactorily addressed. In most of the other developed countries of the world which have developed systems of National Health Insurance, and this includes all of the Western nations except the US, long-term care is a part of the total system. A variety of suggestions have been made ranging from a scheme of National Health Insurance to private insurance for long-term care, tax exemptions on insurance payments for long-term care, and public contributions for the indigent.
