Failure to Consider All Costs of the Operation

Another cause for mistakes is the failure of the decision maker to consider all costs that will be involved in a decision. The decision maker may fail to seriously evaluate all potential costs that could have a negative impact on a project. As an engineer, I always verified the project reductions and the project costs to ensure that savings from the project were realistic and I had another engineer verify all computations related to the project.
