ABSTRACT

This chapter focuses on post-approval of recommendations to be implemented.Now, we need to track if they were actually implemented, and if so, after implementation, did they produced a quantifiable benefit to the company’s bottomline?It is imperative to any effective RCA initiative, it be able to easily demonstrate it achieved (at a minimum) estimated ROIs and impacted associated KPIs as planned.[APG1]

Understanding that in order to have quality outputs, we need quality inputs.What are the quality inputs we are look for in a successful RCA effort?What’s the difference between process measures and outcome measures?Examples are provided from various industries/departments.

An RCA Report Card[APG2], job aid is provided.It helps leadership better assess such RCA quality by checking for the breadth and depth of the RCAs.

Lastly, we discuss the pros and cons of using what is referred to as “zero” metrics.Common terms are zero harm or Zero Failures.Safety Differently advocates are against the use of this metric, and we discuss both sides of the coin, allowing the reader to decide for themselves.
