ABSTRACT
Any investigator can relate to the administrative burdens of organizing an effective Root Cause Analysis (RCA).
In this chapter, we introduce a web-based solution entitled “PROACTOnDemand.” Unlike typical RCA software solutions, this tool is agnostic or approach neutral. This solution is a true investigation management system that helps investigators manage the following tasks: (1) properly executing an evidence collection strategy, (2) manages all aspects of the RCA team, (3) utilizes free-form tools to construct a detailed logic tree while using evidence to support each hypothesis, (4) provides tools for developing a customized report and making the final presentation, (5) develops tracking metrics for each recommendation, (6) trends across the RCA knowledge base for commonalities, and (7) queries an existing RCA knowledge base for suggestions when an subject matter experts (SME) may not be available for the team.
