ABSTRACT

In this chapter, the authors discuss how the core tasks of case management were performed. They describe the practice of tasks related to case management to the community care arrangements. The needs of some fit the provision of one service with sufficient precision for the personnel of the service to perform the case management tasks adequately: the case management tasks required by others can be performed by their families and other professional advisers. The integrated model with workers unambiguously responsible for the core tasks of case management can be contrasted with a 'detached' model in which workers with less clear authority and responsibility might be organisationally separated from the main providers of resources. The integration of the community care team into the divisional structure influenced the choice of procedures for identifying cases. The community care scheme was relocated in a residential home having a high proportion of its beds allocated to short-stay residents.
