ABSTRACT

The County Guide is formatted into a three-phase framework. The design serves as a reference point for each level of service that must be addressed in order for educational outcomes to improve. In order to achieve the most impact on educational outcomes, change in processes must happen not only at the county and systems level but at the individual level as well. Douglas County Human Services, with the support of the action team, created an education direct service position of Educational Navigator. The Douglas County Education Action Team worked on main goals for the first year that incorporated one goal from each phase. The key components summarize a full implementation of the County Guide. It is a final list of components, that once implemented, show successful collaboration of the Education Action Team.
