ABSTRACT

The chapter starts by recognising the impact of life transitions on pupils. The importance of partnership with parents, children, young people and professionals at critical transition points in the pupils’ educational lives is then discussed. Such transition points include: early years and entry to school, transfer within the primary phase and to secondary education, and transition planning at 13+, including transition to Further Education and adult life, in line with the extension of the SEND Code of Practice: 0–25 years (2015) (DfE/DoH). Preparation for adult life is also considered.
