ABSTRACT

This chapter describes an illustrative case study of the practical application of an approach that is intended to foster dialogue around safety among health care stakeholders. The approach, the Safety-II case, is adapted from the safety case used in safety-critical industries, and it aims to provide confidence that stakeholders are reasoning systematically about safety. The chapter argues that, appropriately adapted for use in health care, the safety case approach might contribute towards creating greater awareness of everyday clinical work among different groups of stakeholders. Clinicians, managers and others involved in improving clinical practice can use this approach to take stock of their safety position in an honest and transparent way, and open up their assumptions about safety to review and critique. Goal-based approach has proven to be ineffective due to the fast evolution of technology, and the complexity of modern systems.
