ABSTRACT

In this chapter, the author shows that there are patients who have distinctive difficulties that need a particular understanding and technical approach in analysis. They are what Britton has called "psycho phobic", that is, they are phobic of their own minds and inner lives and fear their unconscious will overwhelm and traumatise their ego. The process of finding meaning, whether it is related to elucidating the transference/countertransference dynamic, uncovering unconscious phantasy, or analysing drive derivatives, is rarely an easy and simple matter, yet most of the time, to some degree or other it can be taken for granted. The two vital aspects of meaningful communication in personal relationships that make this possible are emotional connectiveness and cognitive comprehension, and they usually go hand in hand, one linked with the other, what Melanie Klein often referred to simply enough as "love and understanding".
