ABSTRACT

I EPIDEMIOLOGY Lateral mass fractures of the cervical spine are most commonly caused by extension or flexion injuries of the neck that force the facets into distraction or compression. Facet injuries can elude the inexperienced reviewer on the survey lateral cervical spine x-rays. Their recognition has been enhanced by the advent of computerized tomography (CT) using reconstruction views. Radiographic imaging, however, needs to be combined with clinical evaluation of the patient. Specifically, the posterior column needs to be assessed to avoid missed injuries. Facet injuries include a wide spectrum of ligamentous and/or osseous findings. Their varied descriptions have created confusion regarding their diagnostic and therapeutic recommendations.
