There are a number of situations and body sites in which the results of liposuction may be inconsistent and/or the approach to treatment may present unusual challenges or controversy.

MALE NECK In general, the skin of the male neck is very thick and heavy, with much more fibrous tissue. This is even more of a problem in older men where the skin is not only heavy, but loose. Neck lifts are difficult because the heavy skin tends to fall again. It is quite resistant to trauma. More aggressive instruments have to be used during liposuction. A more aggressive approach, including special maneuvers with the ‘‘smart’’ hand, is used to increase the amount of fat removed. A more thorough fat removal with more unloading of the compartment leads to better skin retraction. Most male necks benefit from this more thorough liposuction. Female neck skin, as well as some

male’s neck skin, is much thinner and lighter and contracts much more easily. Less aggressive liposuction is needed in these cases.
