Today’s progressive pediatric community asks a popular question, “What is the

best state of the art therapy for rhinosinusitis?” Currently, children are taken to

the operating room for pediatric functional endoscopic sinus (FES) surgery

when “maximal medical therapy has failed.” A fair question to ask a surgeon

is “What is maximal medical therapy?” After this chapter defines the mechanism

of pediatric chronic rhinosinusitis, it will challenge the reader to find a more

thoughtfully considered definition of rhinosinusitis and its treatment. The next

pertinent question to be considered, “Is rhinosinusitis a medical or surgical

disease?”
