ABSTRACT

The Connected Communities of Care (CCC) Board was established to bring together leaders responsible for the planning, launch, and expansion of this initiative. The CCC is designed to address patients’ nonclinical needs along with their medical needs in order to significantly improve the patients’ health outcomes. Conflict management plans are necessary due to the diversity and tenure of executives who are part of a CCC. While technology needs will vary by each community and by the specific goals of the CCC, there are several foundational attributes the new CCCs should look for as they select a technology vendor. Data sharing is critical for the success of a CCC and requires that trusted relationships get built across the CCC participants. Clear operational guidelines for data sharing are a core requirement for initially building trust and maintaining it over time. Consistency in approach within sites and across sites is vital for the success of a CCC.
