ABSTRACT

The purpose of the Clinical Providers Track is to set out the stakeholders and processes required to integrate clinical entities, insights, programs, interventions, strategies, and measurement for the Connected Communities of Care (CCC). The sustainability and success of the CCC platform depends on the ability of the Participants to integrate the CCC activities into their daily operations. Leverage Existing Staff Where Possible Staffing constraints can be a common challenge throughout all aspects of the CCC clinical workflows as existing staff are likely already dedicated to multiple projects and are asked to engage in this new effort. The CCC clinical and program leads should work with the frontline staff at clinical sites to determine the most effective times to carry out the pilot of the full clinical workflows and also to assign pilot leads. The CCC should complement existing programs rather than duplicate what is already done or introduce counter-effective activities.
