ABSTRACT

We start this chapter off with a discussion about the hidden treasures which are chronic failures, the unsung heroes of the Reliability world. Preserving chronic failure data is more difficult because most people see them an inconsequential and a “cost of doing business.” However, revealing their annual impacts will change those paradigms! We move on to show a process flow diagram for the PROACT Root Cause Analysis (RCA) system and then identify the 5 P’s of an effective data collection strategy: Parts, Position, People, Paper, and Paradigms. We discuss these data collection categories in detail to help the reader understand the basics of what to collect from a failure scene. All of this wraps up in a neat package by maintaining a Data Collection Form to organize all the data being collected.
