ABSTRACT

This chapter puts into practice what this text has described in practical theory thus far. The case study provided is a result of having the right combination of leadership support, the ideal Root Cause Analysis (RCA) team, and proper application of the PROACT® RCA methodology. During a 2-year period in this North American Paper Mill, there were 19 failures on thick stock pumps. Such pumps typically cost between $60k and $120k/rebuild. Several attempts had been made to implement corrective actions for the pumps, but ultimately, failures were still occurring. This mill applied the PROACT® RCA methodology to this case and were happy to report a 3,500% Return on Investment (ROI), yielding approximate savings of $700k/year off a $20k RCA team investment!
