ABSTRACT

Case management is a complex problem-solving process whose purpose is to obtain, integrate, and coordinate social, psychological, and health services for needy individuals and their families. Although exceptional laypeople are able to coordinate these diverse services, in general the day-by-day process of case management is a task best performed by trained social workers, representatives of the formal system who work in concert with informal networks of family and friends. This chapter discusses the range of services provided by professional geriatric case managers and provides information on the contributors to case management. It presents a case study illustrating aspects of skilled case management at COHME. The chapter also provides a discussion the controversies in the case management field.
