Systems for Tobacco Use Identification and Documentation

Institutionalization of the identification and documentation of tobacco use are key recommendations of the USDHHS clinical practice guideline for treating tobacco use and dependence (Fiore et al., 2000). Institutionalization requires systems-level changes. Systems-level changes, in turn, can enhance the likelihood that clinicians will intervene with patients who use tobacco.
