There is an enormous increase in renal blood flow and glomerular filtration rate (GFR) by the end of the first trimester-of the order of 50%. Tubular reabsorption of sodium and water increases in line with this, to maintain homoeostasis; in fact, some 900mmol extra sodium are retained during the course of the pregnancy. The absolute plasma sodium concentration falls slightly.
