NATURE OF THE DISPUTE: A personal injury claim resulting from a slip and fall in the supermarket.

INITIATOR: Eighteen months ago, the initiator was visiting the supermarket. He was proceeding down an aisle when he slipped on water on the floor. He hit his head and shoulder when he fell. He did not see the sign. He was taken to the emergency room by an ambulance. There were no broken bones, but he did suffer a slight concussion and torn ligaments in his shoulder. Total medical bills for the emergency room and two follow-up visits were USA$24,500. Rehabilitation therapy for five weeks cost USA$21,000. He lost five days of work totalling USA$3,500. He continues to suffer headaches and 10% permanent disability in his shoulder and is seeking a total of USA$505,000.
