ABSTRACT

An application for a certificate is made by a Part 8 claim form (PD 56, para 2.1). The claim form must include the matters required by Part 8, and the following additional matters set out in PD 56, para 5.2:

(a) the nature of the claim or the matter to be determined; (b) the property to which the claim relates; (c) the nature of the business carried on at the property; (d) particulars of the lease or agreement for the tenancy

including: (i) the names and addresses of the parties to the lease or

agreement; (ii) its duration; (iii) the rent payable; (iv) details of any assignment or other devolution of the

lease or agreement; (v) the date and mode of termination of the tenancy; (vi) if the claimant has left the property, the date on which

he did so; (vii) particulars of the improvement or proposed

improvement to which the claim relates; and (viii) if the claim is for payment of compensation, the

amount claimed.
